Student Health & Services Information

South Dakota Codified Law 13-28-7.1 requires students entering school or early childhood programs to present certification that they have been adequately immunized, according to the recommendations of the Department of Health. The law applies to all children entering school for the first time, including transfer students

A Student Health Information form is expected to be filled out by a parent/guardian every year. The individual health history forms are used throughout the school year. The nurse provides a list of medical conditions to the appropriate school staff.

Please notify the school nurse of:

  1. Allergies - especially bee stings, food and medicine

  2. Asthma - even if there is rarely an attack

  3. Any heart defect

  4. Physical deformities - congenital hip, etc.

  5. Surgery

  6. Hearing/Vision problems

  7. Diabetes

  8. Seizures

Medication & Treatment Administration

In the event that your child needs medication while in school, both prescription and over-the-counter, Meade School District requires a completed Medication and Treatment Authorization Form prior to administration. Parent/guardian must complete and sign this form in its entirety prior to the administration of any medications. Any prescription medication or chemical/homeopathic substances or compounds will also require a physician's consent and signature prior to administration.

*Form renewal is required at the beginning of each school year. 

Student Care Plans

If your student has allergies or respiratory issues such as asthma or reactive airway disease, please complete the Respiratory Care Plan and/or Allergy Care Plan and turn in to your student's school health office. Care plan renewal is required at the beginning of each school year.

Dental, Vision & Hearing Services

The school nurses coordinate with the Delta Dental Mobile to set up services every November and February. The school nurses also coordinate with the SDSD Hearing Mobile to provide hearing screenings for identified age groups every spring. Independently, the school nurses complete vision screenings for identified age groups annually.
